to verify your benefits, you must provide your Insurance ID or Social
Security Number, Date of Birth, and telephone number listed on your
insurance card.

Uninsured Patients
For patients who are uninsured, the cost
of the initial visit, which includes an exam, oral cancer screening,
x-rays, and a cleaning is $198. The first visit takes approximately
45 minutes.
